Note: this form is a general outline.  If you have other ideas of what you want on your website please,  use those instead.  If you are going to use you own specific ideas, after filling out the business name and address, etc, you can skip to the bottom of the form where it says additional comments, and just outline your ideas.  Remember this is your site.  Whatever the questions in the form are, remember: If you don't want it on your website, don't put it in the form. The same goes if this is not going to be a merchant site.  If this is going to be an informational site, ie, if you are offering a service, or not planning on accepting credit cards or orders online, just skip the questions that do not pertain to what you are doing.  Government and other organizations are welcome.
 
Note: If you know exactly what you want on your pages you can list pages with exact content, as an alternitive to this form.  List four pages including home page.  We will provide the fifth page(your contact page).

Pictures and graphics 
you are allowed up to 5 pictures for your preview.
 
If you can, send the pictures, and or graphics in pic format, you can also put them in jpg format and they must be executable from the icon, and 72 dpi or less,  include at least 3 product pictures.  Other pictures and graphics can include pictures of store front,  logos etc.
